They lack the trust and personal connection with their professional that is so important for positive interactions and outcomes.
Accessible Professionals’ team includes personalized care coordinators (CARE Advocates) who onboard new members. We would like a team of students to support us in creating an assessment tool/guide that the CARE advocate will use when onboarding new members to help gather information/assess the members’ needs (mental health, post-partum depression, anxiety, anxiety/depression dealing with ill family members/children, etc) and provide recommendations for supports and community resources available. This tool will be used with all new members and will allow us the provide individualized care. It would be helpful for students to research what other assessment/onboarding tools are used
Accessible Professionals is able to share current resources/guides that have already been developed to help the students create the assessment tool.
We will communicate as often as required by the student via email; 12 hour response time. Zoom meetings can be scheduled at the student’s request anytime using our online booking application. During the initial meeting with the student, we will discuss and schedule regular zoom meetings. Students will meet directly with Tyfanny Ross, Co-Founder and CEO of Accessible Professionals.
